A guide created for New Alternatives for Children (NAC) to help foster parents get appointments for themselves for for their foster children

Appointments for foster children

1. Before you call for an appointment for your foster child, make sure that your worker has sent the signed consent form to H+H, or make sure you can bring a copy of the signed consent to the appointment.

 

2. See the list of 28 Pediatric Clinics below and decide where you'd like to go. You can click on the map to link to all the addresses.

 

3. Call the following number for an appointment at any of our 28 Pediatric Clinics:

4. For Dental Clinics: Every clinic has their own phone number for appointments - download the list here.

5. Reproductive Health Services: Many of our hospitals have adolescent clinics; however, if the teen needs additional sites or a procedure like a termination, check out these sites.

 

Appointments for adults

1. If you have health insurance, click on this link to see which of our sites accept your insurance. Scroll through the list or type the name of your plan in the search box. If we accept your insurance, call the number below for an appointment at any of our 50+ sites - ask for Internal Medicine:

2. If you do not have health insurance, call this same number above and choose the option that says "If you don't have insurance or need help paying a bill"

3. The staff will help figure out if should apply for Medicare/Medicaid, or if you qualify for NYC Care, or if you should apply for private insurance.
