A Guide for Agencies to Schedule Appointments at NYC Health + Hospitals
Call for an appointment
1. For children in foster care:
Foster Care Agencies are encouraged to upload these signed forms to EpicCare Link prior to the appointment, but it's not required. The foster parent or case worker can bring these forms to the appointment:

Signed consent for medical treatment

Signed HIPAA release form

You can use your agency's forms, but if you use our H+H TPO form then our health providers can more easily get records from other hospitals.

Foster care intake form

In order to treat this child, we need to know why they are in foster care, and any other information known about their schooling, health, etc.

The agent will take your information, and the child's information and create the chart.

While you are still on the line, the agent will have you upload the 3 documents.

The agent will see the documents immediately and then will assist you in getting an appointment at the Pediatric Clinic of your choice.
2. For families in Preventive Services, the parent must call;

3. For children in foster care:

When the child moves to a new home, please upload this contact information into EpicCare Link so that we know who to contact about appointments or urgent lab results.

For Pediatric subspecialists, continue making those referrals electronically through EpicCare Link.

Reproductive Health Services: All of the FHP sites have reproductive health services; however, if the teen needs additional sites or a procedure like a termination, check out these sites.
