Primary Care for Children in Foster Care and

Children in Preventive Services

A Guide for Staff at NYC Health + Hospitals

How to use this site:
  • This site contains a summary of key information along with the helpful tip sheets for consent issues and how to do things in EPIC.

How agencies will
refer children to us:

1.  For children in foster care:

 

Foster Care Agencies are encouraged to upload these signed forms to EpicCare Link prior to the appointment, but it is not required. The foster parent or case worker can bring these forms:

  • Signed consent for medical treatment​​

  • Signed HIPAA release form

  • Foster care intake form

    • In order to treat this child, we need to know why they are in foster care, and any other information known about their schooling, health, etc.​

  • Note: NYS law says that the agencies can use their own consent forms - they do not need to use H+H's consent forms for treatment or for HIPAA release.

If the foster care staff don't know how to upload document to EpicCare Link, go to this video and share that link with them, or walk them through the process.

  • Hotline hours: Monday - Friday, 9a-5p

  • The agent will take their information, and the child's information and create the chart.

  • While the agency is still on the line, the agent will have the agency upload the 3 documents.

  • The agent will see the documents immediately and then will assist the foster care agency in getting an appointment at the Pediatric Clinic of their choice.

2. For families in Preventive Services, the parent must call the following number to make an appointment:

  • 1-844-NYC-4NYC (692-4692) - call 24/7

3. For children in foster care:

  • When the child moves to a new home, the foster care agency should upload this contact information into EpicCare Link so that we know who to contact about appointments or urgent lab results.

  • To make referrals to Pediatric subspecialists, continue making those referrals electronically through EpicCare Link.
